According to the star’s death certificate, which TMZ revealed on Tuesday, Welch died of heart arrest. The two-time Golden Globe nominee also had Alzheimer’s at the time of her death.

Welch was declared dead at 2:25 a.m. on Feb. 15 and cremated, according to Page Six .

Welch At the time, her manager Steve Sauer and management, Media Four, issued a statement to The Los Angeles Times saying, “Raquel Welch, the legendary bombshell actress of film, television, and stage, passed away peacefully early this morning after a brief illness.”

The statement went on to say, “Her career spanned over 50 years starring in over 30 films and 50 television series and appearances. The Golden Globe winner, in more recent years, was involved in a very successful line of wigs.”
